Essential Forms for Enrollment and Participation in Our Programs

All prospective and current students at Five Star Equestrian are required to complete the correct forms before joining our programs. Whether you are enrolling in the Riding Academy, signing up your child for summer camp, or reviewing our billing policies, this page gives you direct access to the documents needed for registration. Each PDF is updated regularly to match our latest procedures and to help you get started in our structured, goal-focused equestrian setting. We do not provide recreational trail rides, and all forms are specific to our competitive and educational offerings. If you are unsure which forms you need, please call us at 650-440-2450. Summer Camp 2026 Registration

Required for all participants in our seasonal horseback riding camps (ages 5+). Submit this form to reserve your child’s spot in our hands-on, educational day camp.

Note:

If you are enrolling in the Showteam, Hunter/Jumper Training, Technical Instruction, or Girl Scout Program, additional paperwork may be required and will be provided after your initial consultation.
